Action Kids Athletics Release Policy

~ Action Kids Athletics requires a release/registration form for all participants under the age of 18.

~ The release must be signed by the parent or legal guardian of the participant.  

~ Step parents, aunts and uncles, grandparents, "close family friends" etc, cannot sign for the child, unless recognized as a legal guardian by the court.

~ We do not accept verbal releases from the parents.

~ Hand written notes are not acceptable. 

~All releases must be completed and signed in ink.

~ Releases for birthday parties and "Bring a Friend" are event-specific. Your child must bring a release to each birthday party or Bring A Friend class they attend.  If you have misplaced your Bring a Friend or Birthday invitation (which includes a release), you can download our general registration form below.

~ We require all releases to be updated annually.
